Detection of diabetic cardiomyopathy in Egyptian children and adolescents with longstanding Type 1 diabetes and evaluating the effect of α-tocopherol supplementation on cardiac functions after 1 year; a single center prospective study.

Introduction Diabetic cardiomyopathy (DCM) is a serious complication that frequently occurs in patients with type 1 diabetes (T1D) necessitating early diagnosis. The aim of the current study was to detect subclinical DCM Egyptian children and adolescents T1D evaluate effect antioxidants on myocardial dysfunction. Material methods prospective observational cohort included 81 (9-20 years old) duration > 4 compared 50 age sex matched non-diabetic controls. Serum N-terminal prohormone brain natriuretic peptide (NT-proBNP) assessed addition conventional echocardiography, tissue Doppler imaging (TDI) speckle tracking echocardiography (STE). Patients dysfunction were supplemented vitamin E (α-tocopherol) as an antioxidant for year, then follow up done. Results NT-proBNP elevated 73 (90.1%) patients, 50.6% had left ventricular (LV) diastolic dysfunction, 14.8% right (RV) 84% LV systolic 91.4% RV statistically significant difference There negative correlation between global longitudinal strain (GLS) (p<0.001, r=-0.796). ROC curve analysis showed at cut-off point ≥ 195 ng/ml detected function 89.7% sensitivity 84.6% specificity. Follow after one year α-tocopherol supplementation revealed improvement functions well Conclusions non-conventional useful detection T1D. Antioxidant treatment improved patients.
